Community Care
Providing high quality residential accommodation for those unable to care for themselves is only one part of Churches of Christ Care’s mission to help the Queensland seniors and their families. Our dedicated and caring community aged care staff support seniors living at home to remain independent and enjoy all their community offers.
We deliver a number of community based services across Queensland which are designed to assist the elderly remain independent safely and with a high quality of life. These services include:

Community Aged Care Packages
A Community Aged Care Package (CACP) are government subsidised packages of care, offered to older people who wish to remain at home, but require assistance with some day-to-day tasks.
CACPs provide assistance with activities such as:
- personal care – bathing, toileting
- housekeeping
- meal preparation
- social activities – at home and in community
- transport to appointments
- rehabilitation programs.
Packages are individually tailored to meet each client’s changing needs.
How do I receive a community aged care package?
Firstly, you need to be assessed by an Aged Care Assessment Team. Your local GP should be able to point you in the right direction. Then, if a Community Aged Care Package is appropriate, you will be referred to a service such as Churches of Christ Care.
Consumer Directed Care (CDC)
Consumer (or self) directed care allows older people who require in home care to have greater control over their own lives by supporting them to make choices about the types of services they need and how they are provided. The service provided as part of CDC low care are designed to meet an older person’s needs such s shower assistance, meal preparation, laundry, home help, gardening, medication support and transport. A coordinator will assist you to plan the service that best meet your needs once you have been assessed by an Aged Care assessment Team.
Extended Aged Care at Home – EACHD
What is an EACHD?
Extended Aged Care at Home Dementia (EACHD) packages are individually planned packages of care tailored to help older Australians who experience difficulties in their daily life because of behavioural and psychological symptoms associated with dementia, such as significant memory loss, agitation and depression.
Why EACHD?
EACHD offers you and your loved one who lives with dementia greater quality of life and independence. Several of the key benefits of an EACHD package are:
- your loved one remains independent at home with enhanced quality of life
- premature entry into residential care is prevented
- support for carers with practical help is provided so they can receive a break.

Healthy Ageing Program
The Healthy Ageing Program is run by older people, for older people, with the assistance of a coordinator. The program aims to encourage older people in the community to take action to improve and maintain their health and wellbeing in their retirement years.
It focuses on involving older people in making healthy decisions and ensures people receive relevant, accurate and appropriate health-related information. It offers various activities to keep the body and mind healthy. Some of these activities include:
- social get-togethers
- gentle exercise
- aqua exercise
- bus trips
- guest speakers
- games
- craft
- walk and talk.
Participants meet regularly to plan fun activities, discuss health issues and decide on directions for the program.
Funding is provided by Queensland Health and Churches of Christ Care operates this program for older people in the Balonne Shire in Western Queensland. The program is one of over 25 Healthy Ageing Programs throughout the state.

Home Assist Secure (HAS)
Difficulty with home maintenance, repairs and modifications have often been identified as major housing related problems for older people and people of any age with a disability. The State Government’s response to these needs has been the introduction of the Home Assist Secure (HAS) program. HAS aims to improve the safety, independence and quality of life for people 60 years and over and people of any age with a disability, who wish to remain living in their own homes. Home and Community Care (HACC)
Home and Community Care (HACC) Services provide a range of services to support older people, and younger people with a disability, to remain living in their own home.
The program aims to promote independence and avoid premature entry into long-term residential care.
HACC Services may include:
- Personal care – including showering, dressing, toileting and other self care tasks
- Respite care – to provide the carer with a break from their caring duties
- Social support – helping clients with shopping, banking, outings, visits to friends and family
- Nursing service – blood pressure, blood sugar level and dressing checks
- Transport to appointments/shopping.
- Home maintenance, such as changing light bulbs or tap washers and minor general repairs (with selected HACC Services)
- Domestic assistance, including cleaning, washing, ironing, vacuuming (with selected HACC Services)
- Allied Health Care – podiatry, physiotherapy and other allied health services according to a needs based assessment
- Centre-based Day Care – longer periods of respite care for a number of HACC eligible persons to provide primary carers with a good break
- Case Management – coordination of required care for HACC eligible persons who need support to access multiple services and support to understand the health care system
HACC is a joint Commonwealth, State and Territory initiative.

Older Peoples Action Program
We have established the Older Peoples Action Program – Safe and Confident Living Project, which operates from Golden Age Retirement Village on the Gold Coast. The program works intensively with individual clients, referred to the program, who are at high risk or are already experiencing social isolation and loneliness.
The role of the program is to work with older people living in the Gold Coast City to enjoy a safe and confident lifestyle. We do this by:
- maintaining an up-to-date knowledge of community services and agencies which support older people to live safely and with confidence
- providing a support service which responds to the social issues affecting older people
- increasing older people’s knowledge and use of community resources available through short courses and information sessions on topics such as being safe at home and in the community, using ATMs etc
- encouraging older people to maintain or extend their levels of participation in the local community
- working with older men, teaching them new skills such as ’Cooking for One or Two’ courses, One Person CPR, Older Drivers Safety Courses etc.

TravelCare
We offer home based respite for primary carers of persons suffering dementia, behaviours of concern and inappropriate behavioural disorders which restrict their access to generally available services and/or refuse to attend Day Respite Centres. This service is able to be offered through the Commonwealth Government’s National Respite for Carers Program.
Home based respite is offered in blocks of time, for example 2-6 hours, to enable primary carers to catch up with other family members, friends, shopping and/or just to have time out.
Our staff work in consultation with the primary carers to initiate a program to provide coordinated services which are reliable and planned to meet individual, ongoing, specific needs, with flexibility for clients in Beaudesert and surrounding areas.
Some activities TravelCare staff can assist with include daily task of personal hygiene, transport to respite day centres, appointments to doctors and other professionals.
Through this program, Care staff can also offer assistance to locate appropriate facility respite and other services as required.
TravelCare also offers clients monthly bus trips, pamper days, luncheons, and craft days. Home base respite will be arranged to allow primary carers to attend carer support monthly gatherings.
